It only takes one guy determined never to retreat, to guilt the rest of the group into hanging round. If the GM then saves the party from all the TPKs that would result, never retreating becomes the logical step and then a habit.

The main reason to do a boxed set - aside from just doing something that the last marketing guy didn't, which is still my bet - is so you can drop everything that a new roleplayer might want to start with. I'd assume that in both cases the people in charge of marketing at first didn't like the idea, then later the marketing people were replaced for whatever reason and the new guys decided to try something else. I've seen stuff like this happen where I work (not in gaming).

Wouldn't this depend on what sort of SR campaign you were in? I mean, Masking is useful if you want to emphasise the staying in the shadows part, but not so much in a Pink Mohawk style campaign. Vice versa for Invoking.

Practically any sort of buffing could do this, from spells and/or bardic music, to the druid wildshape variant which lets you sprout fangs.

There's a feat in ToB (Stormguard? something like that) which lets you make nondamaging attacks in exchange for getting bonuses on the attacks in the next round.
