I have adjusted Fade Out to have a 1/day per 3HD usage with duration of 1 round/HD.
The wraithblade form is elaborated and clarified.
Does this help?

Not really.
Fade Out is now too good at 5th level. While you can only use it once per day at that point it still lasts the whole combat and now doesn't have the drawback of needing a move action each turn. It still lets you fly at 50ft with perfect control at level 10. It's still a "Fuck You" to all melee creatures.
Look, at 5th level there's already an example of an incorporeal creature that phases back and forth and fights in melee: the Phase Spider. It can't attack while incorporeal and can be attacked with a readied action. Don't make the incorporeal ability any better than that, and probably worse until about level 7. I don't know, something like make a Hide check with a -10 penalty as a move action each turn, and if you succeed you are incorporeal to the creatures that fail. Just a random idea.
Seriously, try the same game test and imagine how the Ghost Blade is supposed to fight creatures. I've copypasted it below, and even added some links to the SRD to make it easy. The Fade Out ability makes it win against everything on the EL 10 list three times a day, and against most of the EL 5 list once per day. It's too good.
Wraithblade is better worded now though.

Oh by the way Parthenon, I'm not ignoring you or something, just very busy. I promise I will try to fix this up as soon as possible and get back to you when I can.
